Please prepare the following the documents:
1. Accomplished registration form. See below.
2. 2 x 2 ID picture with white background
3. Updated Curriculum Vitae
4. Photocopy of your PRC ID (please bring original for authentication)
5. For nurses, a photocopy of your PNA ID
6. A P1,000.00 deposit to any BDO branch
- Account Name: Association of Diabetes Nurse Educators of the Philippines
- Account Number: 300-253-257.
If the payment was made through BDO, please inform us via SMS or fax the deposit slip with your name to (02) 942-0313. The P 1,000.00 deposit will be deducted from the total P 9,000 training fee upon full payment. If you decide not to pursue the training, only 500.00 will be refunded to you.
